To complete the form you can print out a copy and hand it to your vets directly, or send it to them via email, using the link above. Or, you can contact me with your details if you prefer, and I can contact them myself. Your vet may want to see you beforehand for an appointment, however this is not a necessity, it is up to their discretion if they wish to do so. A signed, completed form certifies that your vet has given permission for me to perform physiotherapy treatment for that animal.

Follow-up appointments: £50 equine patients and £40 canine patients
Full day (yard/kennels): £300
Charges are based on initial appointments at 90 minutes, follow-up appointments 60 minutes for equines and 40 minutes for canines, all variable, subject to the individual case.

If you wish to book an appointment and are based outside of the circled area we will discuss beforehand the additional travel costs I will have to add on to the cost of your appointment.
